PSYCHIATRIC EVALUATION AND MEDICATION MANAGEMENT
Michelle Miranda believes that medication is only part of the equation in treating mental health concerns, and will work with you in exploring a variety of treatment modalities for best results. She provides a comprehensive initial psychiatric assessment and ongoing follow-up treatment for the management of symptoms, all via telehealth for ease of access. She has extensive experience in the treatment of depression disorders, bipolar disorders, substance abuse disorders, anxiety disorders, and psychotic disorders. Michelle provides medication assisted treatment for substance abuse disorders, including Suboxone/buprenorphine for opioid use disorders. She works with clients ages 16 and up, is LGBTQ friendly, and welcomes clients from all walks of life.
PAYMENT
A valid credit card is required to secure your appointment. This card will only be charged for private pay appointments, at the time of the appointment, for additional agreed upon services, or for missed appointments/late cancellations. You may always elect to pay with an alternative source. Fees for services already rendered will not be refunded. Michelle Miranda uses a HIPAA compliant merchant service for payments. Michelle is able to accept all major credit and debit cards, including FSA and HSA cards, for services.
INSURANCE AND REIMBURSEMENT
Crescent Moon Psychiatry is currently able to accept clients with the following insurance providers:

For all other insurance providers, we are considered “out-of-network”.
We are unable to accept Medicaid or Medicare clients.
Clients without insurance or with alternative insurance carriers are always welcome. Private pay clients are expected to pay the full fee for services rendered. Depending on your insurance carrier, you may still qualify for reimbursement for services rendered. Upon request, Crescent Moon Psychiatry can provide you with a billing statement for services rendered so that you may file an “out-of-network” provider claim (or “superbill”) to submit to insurance for consideration of reimbursement.
Crescent Moon Psychiatry cannot make any guarantees about how much you may be reimbursed by your insurance carrier. If you wish to pursue reimbursement, we recommend that you contact your insurance carrier and asking the following questions to determine your benefit eligibility and coverage as well as limitations and out of pocket costs:
• Does my health insurance plan include mental health (medication management) benefits?
• Do I have out-of-network mental health benefits?
• Is any pre-approval required before obtaining out-of-network mental health services?
• Do I have a deductible? If so, what is it and have I met it yet?
• If I have out-of-network mental health benefits, will I be reimbursed the full amount I paid, or a portion of it?
• Does my out-of-network plan limit my psychiatric sessions per calendar year? If so, how many sessions are allowed?
• What information and requirements need to be submitted for reimbursement?
