WHAT IS A PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER (PMHNP)?A Psychiatric Mental Health Nurse Practitioner (PMHNP) nurse who has received advanced education (Master level or higher) specifically in psychiatric and mental health disorders. A PMHNP's role is to assess, diagnose, and treat mental health disorders. This includes authority to prescribe medications to treat mental health disorders. In the outpatient setting, there are very few differences in the types of services that can be provided by a PMHNP versus a psychiatrist (an MD or DO specializing in psychiatry). One primary difference between a psychologist and a PMHNP is that a psychologist cannot prescribe medications. While Psychiatrists and PMHNPs typically have a stronger focus on medication management during session, a psychologist typically has more background in counseling, behavioral interventions, and psychological testing.

SHOULD I ALSO BE IN COUNSELING/THERAPY?In most cases, yes. Michelle Miranda believes that having a counselor/therapist to help you process different concerns in your life is key to improving your mental health. Medication will only help so far if you are not addressing underlying, and often deep-seated, issues. A counselor/therapist is an important tool to be able to access during your mental health journey and Michelle is happy to make recommendations at your request.

WHERE IS YOUR PRACTICE LOCATION?Crescent Moon Psychiatry is located within the offices of Solaris Integrative Psychiatry. Our address is: 1302 S. Shields St. Unit A2-4 Fort Collins, CO 80521 Directions: From Shields St., turn east on Springfield Dr. and park in the lot to the left. We are located at the south entrance to the building, on the second floor, to the left of the stairs. There is a sign at the door for "Solaris Integrative Psychiatry", with whom we share office space. Please have a seat in the lobby and enjoy the music; Michelle will escort you to the evaluation office for your scheduled appointment! ***By appointment only, walk-ins are not accepted.***

WHAT IF THERE IS A PSYCHIATRIC EMERGENCY?If you or someone you know is experiencing a mental health or medical emergency, please call 911 or go to the nearest emergency department. HOW DO I MAKE AN APPOINTMENT?You may book your appointment through the "Book Now" page on this website. You may also contact Crescent Moon Psychiatry via phone to book with Michelle directly at (970) 427-2820. We ask that you do not use email to request bookings.

HOW LONG ARE APPOINTMENTS?Psychiatric Initial Assessment appointments are scheduled for 50 minutes. Psychiatric Follow-Up appointments are scheduled for 30 minutes. All appointment times are strictly maintained and appointments cannot be extended due to late arrivals.

HOW FREQUENTLY WILL I NEED TO SEE YOU?This varies by person and most commonly ranges from every 1 to 3 months. If we are making medication adjustments, you may need to see Michelle Miranda more frequently. Often, the first several months of treatment may require more frequent appointments.

HOW DO I PREPARE FOR MY FIRST APPOINTMENT?Upon booking your first appointment, you will be sent further information via email. This will include a questionnaire about your past and current mental health and medical treatment. These forms must be completed within 48 hours of booking in order to confirm your appointment.

HOW ARE APPOINTMENTS HELD?Appointments are offered in-person and via telehealth. In-person appointments are held at: 1302 S. Shields St Unit A2-4 Fort Collins, CO 80521 For all telehealth appointments, you will be required to sign a telepsychiatry consent form to engage in virtual treatment with Crescent Moon Psychiatry. Crescent Moon Psychiatry uses a HIPAA compliant telehealth communication system which you will be able to access directly in your appointment confirmation email. You will not need to download anything, just allow camera and video access. Please note: voice and video are required for telehealth appointments, and the client must be currently located in the state of Colorado.

WHAT IS TELEPSYCHIATRY?Telepsychiatry (or telehealth) uses modern technology to conveniently allow access to your provider virtually from the comfort of your own home (or office). All telehealth appointments with Crescent Moon Psychiatry are held using a secure and HIPAA compliant application. For confidentiality reasons, we ask that you ensure that you are in a private location for your scheduled appointment time. Voice and video are required for telehealth appointments.

WHAT IF I HAVE A BAD CONNECTION DURING MY TELEHEALTH APPOINTMENT?We ask that you ensure that you have a private location with a secure internet or data connection prior to your scheduled appointment. Appointments missed or significantly impaired due to faulty connection on your behalf will not be reimbursed.

DOES IT MATTER WHERE I LIVE FOR TELEPSYCHIATRY APPOINTMENTS?You may live outside of Colorado but you must be located in Colorado at the time of your appointment to be seen by Michelle Miranda via telepsychiatry. If using insurance, you may be required to provide additional information before being seen. Please see the "Payment" FAQ section below for details about using your insurance.

WHAT IS YOUR CANCELLATION POLICY?Crescent Moon Psychiatry requests that clients make changes to or cancel their appointment at least 2 days prior to the appointment start time. Except on a case-by-case basis for personal emergencies, changes or cancellations made with less than 48-hour notice from the appointment start time will incur a fee. If you must cancel, please contact Michelle Miranda directly by email at info@crescentmoonpsych.com, phone at 970-427-2820, or via the patient portal. Clients are expected to arrive to their appointment on time. If you arrive to your appointment late, you will be seen for the duration of the time originally allotted for your appointment (ie if you booked a 30 minute appointment and arrive 7 minutes late, you will be allotted 23 minutes for your appointment). If a you arrive more than 15 minutes late for your psychiatric initial assessment or more than 10 minutes late for your psychiatric follow-up appointment, the appointment will be considered cancelled and the cancellation fee will apply. WHAT FORM OF PAYMENTS DO YOU ACCEPT?Crescent Moon Psychiatry is able to accept all major credit and debit cards, including FSA and HSA cards, for services rendered.

DO YOU ACCEPT INSURANCE?Crescent Moon Psychiatry is currently accepting clients with the following insurance providers: Anthem Blue Cross Blue Shield of Colorado Aetna Cigna Oscar Health Oxford United Healthcare Crescent Moon Psychiatry uses Headway, a HIPAA compliant insurance processing company that assists with insurance verification, billing and claims processing, and invoices. You may create or access your account here: https://headway.co/. Clients are responsible for maintaining/updating their insurance information. If insurance information is not accurate within the Headway portal at the time of a scheduled appointment, the credit card or payment form on file will be charged the full appointment amount. For all other insurance plans, Crescent Moon Psychiatry is considered “out-of-network” (OON). We do not process or submit bills with OON insurance companies on the client's behalf. Clients with OON benefits or without insurance are expected to pay the full fee for services rendered. Depending on the client's insurance plan, the client may qualify for reimbursement for services rendered. Upon request, Crescent Moon Psychiatry can provide the client with a billing statement for services rendered so that the client may file an OON provider claim (or “superbill”) to submit to insurance for consideration of reimbursement. Crescent Moon Psychiatry cannot make any guarantees about how much a client may be reimbursed by the client's insurance carrier. If a client wishes to pursue reimbursement, we recommend that the client contact their insurance carrier and asking the following questions to determine their benefit eligibility and coverage as well as limitations and out of pocket costs: • Does my health insurance plan include mental health (medication management) benefits? • Do I have out-of-network mental health benefits? • Is any pre-approval required before obtaining out-of-network mental health services? • Do I have a deductible? If so, what is it and have I met it yet? • If I have out-of-network mental health benefits, will I be reimbursed the full amount I paid, or a portion of it? • Does my out-of-network plan limit my psychiatric sessions per calendar year? If so, how many sessions are allowed? • What information and requirements need to be submitted for reimbursement?

DO YOU ACCEPT MEDICAID CLIENTS?Crescent Moon Psychiatry is out-of-network with Medicaid and is unable to accept out-of-pocket payments from Colorado Medicaid or other Medicaid members for any services. Providing a personal payment for services to Crescent Moon Psychiatry while a client is enrolled with Medicaid can put the client's eligibility for continued Medicaid coverage at risk of being cancelled. By continuing with Crescent Moon Psychiatry, you are acknowledging this exclusion and confirming that you are not a member of Colorado Medicaid or any other Medicaid program.

HOW DO I PAY FOR SERVICES?For clients using insurance, Michelle Miranda uses Headway, a third party insurance processing system. Headway is used to to bill insurance and will charge the client directly for any co-payment or other fees due at the time of the scheduled appointment. Headway will maintain a card on file for billing purposes. All clients booking an appointment with Crescent Moon Psychiatry will be required to enter a credit card to secure their appointment. If paying out of pocket, the appointment service will be charged at the time of the scheduled appointment. The credit card on file will only be charged for agreed upon services (ie appointments or other services requested), and cancellation fees. Cancellation fees and fees for services already rendered will not be refunded.
