A credit card is required to secure any appointment. Your credit card will not be billed except for agreed upon services and late cancellations, per my cancellation policy.
Intake forms will be sent to the email you provide after you book an appointment. These forms must be completed within 48 hours of receipt in order to confirm your appointment. Your appointment is NOT confirmed until the intake forms are completed and submitted. Failure to submit these forms will result in the appointment time being released. You may rebook when you are able to complete the required forms.
